Adult Day Health Care Centers
The Adult Day Health Care programs operated by Health for All are a combination health clinic and social service center for the frail elderly or mentally and physically disabled adults from eighteen years and older. Each facility is a multi-ethnic community dedicated to providing seniors and underserved adults the opportunity to remain healthy and active for as long as possible. By fostering the highest level of functioning for the individual, the staff works to improve the quality of life and to prevent, or, at least, delay institutionalization.
Health for All provides health and social services to stroke patients and adults with chronic illness, head trauma, mental illness, or those who need assistance with daily personal needs. The programs improve the quality of life for seniors who are deteriorating due to social isolation and/or minimal activity, and help restore or maintain optimal functioning capacity to the frail or disabled elderly. Alzheimer patients are provided a safe and therapeutic environment. In addition to providing group activity and a therapeutic social environment for the participant, Health for All's Adult Day Health Care program offers the family caregivers a needed respite from continuous care of their adult dependent. The Centers provide preventive medical care, health education programs, transportation, dietary assistance, hot lunches, speech/occupational/physical therapy, social and recreational activities, and psychological counseling.
Services are reimbursed by Medi-Cal or through private payment that, in cases of real need, is based on a sliding scale. The current Medi-Cal reimbursement is $67 per day. Adults who require health care but are not totally bedridden are eligible for services. Individuals in wheelchairs are welcome! There are four Adult Day Health Care Centers, located in Auburn , Lodi , downtown Sacramento , and the Meadowview are of South Sacramento .
